Fidelis Care is proud to provide you with comprehensive service and care through our Fidelis Medicaid Advantage Plus program, now available in the five boroughs of New York City, and in Albany, Rensselaer, Schenectady, and Montgomery counties. As a member, you will have your own unique care plan that is coordinated by a Care Manager. He or she will oversee your care, make appointments, arrange for transportation, and coordinate all services with you, your family, your doctor, and other health care providers and caregivers in your community. Our office hours are 8:30 a.m. to 5:00 p.m. Monday through Friday. During evenings and weekends, if you need immediate assistance, a Care Manager is on call 24 hours a day.
