Fidelis Care at Home is committed to providing members with the care and support services that they need while maintaining the dignity and independence that they deserve. Members of Fidelis Care at Home receive individual attention and flexible care planning to help them to live independently in their own homes and communities for as long as possible. Every member's individualized care plan is overseen by the member's personal Nurse Care Manager. The Nurse Care Manager evaluates each member's healthcare needs, and creates a plan of care that is fully coordinated with the physician, the service providers, the member, and the member's family. The key is to help members achieve their personal healthcare goals by providing quality care and services.
Fidelis Care at Home recognizes that the needs of members are varied; therefore, a wide range of services are available to suit individuals and their circumstances:
- Personalized plan of care coordinated with physicians and other healthcare providers
